DIABETES MANAGEMENT NURSE

Loyola University Health System, located 10 miles west of downtown Chicago, is currently seeking a RN-CDE with 3-5 yrs. of intensive diabetes management of patients including: prior exp. with insulin pump management, CGMS, and diabetes education (both group & one-on-one). Current CDE and RN license required. Practice consists of seven well-established busy physicians and a Nurse Practitioner. Patients will be seen at multiple locations.

Staff Nurse- RN; Endocrinology- Glen Ellyn

 
Position Description

This newly created position REQUIRES a minimum of one year related nursing experience. Candidates with blood glucose testing and insulin pump technology experience will be given preferance.
Position involves heavy patient education and phone triage. Ability to perform test ordering, grow in technology requirements, and anticipate needs of the patient & department.
Position Summary:
Under the direction of Site Physician(s) and Practice Manager, responsible for assessment and treatment of patients in an outpatient/office setting.  Provides treatment and follow-up education consistent with patient’s age and cultural needs.     Responds to all patient inquiries, prioritizing responses and routing urgent situations to a physician, PA or NP (hereafter Provider).   Maintains accurate and appropriate documentation and follows patient privacy in accordance with HIPPA and DMG policies and procedures.   Acts as role model for all other care givers in the coordination of patient care.   Performs to the level of a licensed registered nurse or practical nurse as dictated by the Illinois Nurse Practice Act and DuPage Medical Group.   
Patient Assessment and Treatment:
Consults and coordinates with health care team members to assess, plan, implement, and evaluate patient care.  Assists Provider in taking patient histories, obtaining vital signs, and preparing patient for routine exams or procedures, including collecting and processing specimens.  Observes, records and reports the patient’s condition and reaction to treatments and/or drugs.  Where appropriate, administers first aid, immunizations and injections; assists Provider with minor surgical procedures, including the application and/or removal of casts, posterior molds and splints, sutures and staples.    Administers prescribed medications and treatments in accordance with approved nursing techniques and as directed by the Provider; including oral, cutaneous (i.e., patch), intramuscular, and subcutaneous and IV routes (RNs only).    Prepares and orders tests: fills out lab requisitions, test orders, patient forms and referral forms as per the Provider’s request.  Tracks and obtains all test results for Provider review; notifies patients of results as directed.   Assesses the patient’s physical, age specific, and psycho-social needs as related to the disease process, treatment, and education.    

Patient  Documentation:
Maintains accurate and detailed patient records, charts and other pertinent information; posts tests and examination results.  Ensures medical chart/lab test results are available and complete and all records are appropriately secured in the patient chart.   

Patient/Family Education:
Delivers patient education, according to the individualized needs of the patient and as prescribed by the Provider and/or Group policy.  Educates patient and/or family members about diagnostic procedures, medications, nutrition and maintenance of health and wellness.   Responds to patient questions and appropriately provides reassurance while responding to patient concerns.

Triage Phone Response:
Responds to/refers incoming patient treatment-related phone calls. Triages patient messages and responds in accordance with departmental policy and procedures.  May assist in arranging tests or admissions, calling in prescriptions, etc.  Reviews and prioritizes same day messages.   Under direction of the Provider, RNs can assess and determine patient needs… LPNs can convey symptoms to a Registered Nurse, physician,   PA or NA for best course of action.

Safety Procedures:
Responsible for routine maintenance and preparation of medical equipment; cleans, disinfects, and/or sterilizes instruments and equipment following CDC safety and OSHA standards.  Follows Group policy and OSHA guidelines for handling of blood and body fluids and uses PPE appropriately.  Maintains quality assurance records of the disinfection process; serves as a role model of safe office practice.

Clinical Resource for Staff:
Performs and delegates a variety of tasks to facilitate patient office visits and procedures.  Aware of coding and reimbursement issues in delivering care.   Available to other office clinical and non-clinical staff to answer questions and provide assistance as appropriate.  Assists in orienting and training new staff.   Seeks opportunities to improve or enhance nursing and leadership skills.


Other Duties as Assigned:

Maintain Medication Count:  
Ensures narcotics are secured and appropriately accounted for on a daily basis.   Inventories drugs and relays or orders additional supplies.  Ensures appropriate transport of expired medications to the centralized mail room.

Schedules Tests/Procedures:
May schedule tests and procedures and obtain insurance pre-authorization.

Position Requirements

Education and/or Training Required:  

Graduation from an accredited Registered Nursing program.  Knowledge of nursing skills in accordance with nursing standards in the Nurse Practice Act.  Must have a current CPR certification and a current and valid Illinois Nurse License.

Experience Required:

Diabetes Educator-RN

 
Position Description

Rare opportunity for an experienced Diabetes Educator with with IL RN licensure. This is a full time position M-F -60% of time in Glen Ellyn and 40% of time in Bloomingdale/Stratford.  M,W & F: 9am-5pm and T &Th: 10am-7pm. You will educate diabetic patients with disease state and educate them on life behaviors and the start up of medications. Group education skills and Insulin Pump knowledge & certification required. Serve as a consultant for patients and staff, provides direct Type I, Type II, ambulatory insulin and gestational diabetes self-management education to patients and is responsible for creating, coordinating and evaluating outpatient programs relative to diabetes.


DESCRIPTION  
Primary Duties and Responsibilities:

Facilitates collaboration among interdisciplinary team members in the plan of care for diabetes patients.

Functions as a consultant, educator, program planner and practitioner in the outpatient setting.

Assists Endocrinology Dept. in providing pre-pump assessment & education and insulin administration/education.

Educational interventions should include: an initial assessment of health history, resource use, risk factors, cultural influence, health beliefs and behaviors, support systems, socioeconomic factors and learning barriers.

Documents the essential assessment, instructional content, response to interventions and the plan for follow-up in the medical record.

Communicates/collaborates with PCP regarding patient progress. Identifies issues in delivery of diabetes management.

Utilizes patient data, current research and goal setting to meet educational needs of diabetes patients and their significant others.

Assesses, interacts and cares for patients in a manner that demonstrates understanding of the cognitive, physical, emotional and chronological maturation process.

Demonstrates practices, which are consistent with professional associations, hospitals and/or clinic-based standards of care.

Establishes relationships with representatives associated with diabetes care to maximize knowledge base of new strategies for diabetes management.

Completes the mandatory hours of continuing education established by the American Association of Diabetes Educators to maintain Diabetes Educator Certification.

Supervisory Responsibilities:
Currently does not have any supervisory responsibilities.

Position Requirements

ADDITIONAL REQUIREMENTS  
General Skills and Abilities Required:
Must be experienced in PC/keyboard skills and Microsoft office applications. Must possess excellent communication and writing skills. Have ability to plan, organize, and work independently. Exhibits problem-solving skills. Maintains current knowledge of up-to-date American Diabetic Association recommendations. Able to develop professional rapport with employees at all levels of the company.

Special Skills or Licensure Requirements:
Registered Nurse with current license from the Illinois Department of Education and Registration. Current certification through the American Diabetic Association (Certified Diabetic Educator). Must have current CPR status. Group Education skills and Insulin Pump knowledge & certification required.

Education and/or Training Required:
Registered nurse with a Bachelor’s degree from a four-year college or university or equivalent combination of education and experience.

Experience Requirements:
Four years clinical nursing experience with at least 2000 hours of which must be in the area of direct diabetes patient education. Experience in program management, with knowledge of American Diabetes Association Program Recognition Standards.

Typical Physical Demands:
While performing the duties of this job, the employee is regularly required to listen attentively and speak clearly and concisely. Frequent mobility and/or sitting for required extended periods of time. Some bending and stooping required. Requires occasional lifting up to 30 pounds of boxes or paper. Requires manual dexterity to operate keyboard, calculator, photocopy machine and other office equipment. Requires eyesight correctable to 20/20 to read numbers, policies, and computer terminals. Requires hearing within normal range for telephone use. Must be customer-focused.

Pump training is a plus but will train.
